I further release and agree to indemnify and hold harmless Rochester Youth Camps, LLC, the Rochester Americans Summer Hockey Camp, the Rochester Rattlers Summer Lacrosse Camp, the Two Sticks Hockey/Lacrosse Combo Camp, the Spotlight Theater Camp, The Rochester Americans Hockey Club, the Rochester Rattlers Lacrosse Club, its employees, agents and camp staff and facilities from any and all liabilities for injuries or loss of property incurred while attending the camp and further agree that the use of all Rochester Youth Camps, LLC, facilities is made at the risk of the applicant. The camper and parent agree that any pictures taken by the camp can be used in any promotion or advertisement for the camp. I hereby authorize the staff of Rochester Youth Camps, Rochester Americans Summer Hockey Camp and the Rochester Rattlers Lacrosse Camp to act for me according to his/her best judgement in any emergency requiring medical attention and, if necessary, seek and provide appropriate medical attention for the camper. I will be responsible for the costs of medical attention and treatment. I hereby waive and release the Rochester Americans, Rochester Rattlers, Rochester Youth Camps, its employees and camp staff from any and all liability for injuries or loss of property incurred.
